I have always wondered why Mt. Vesuvius isn't on the list - not even on the Italian tentative list. It's probably the most famous and significant volcano anywhere, it's geologically important and of great scenic value (even if the access roads are full of trash). Also, the historical importance is unmistakable. Maybe a site in combination with Pompeii and the Phlegreian Fields (which are on the T list)?
