Nasca - Indiana Jones and Kingdom of the Crystal Skull
Petra - Indiana Jones and the last Crusade. The "Canyon of the Crescent moon" was Al Khaznah, The Treasury
Venice - Indiana Jones and the Last Crusade
Granada Indiana Jones and the Last Crusade. The Alhambra doubles as the Port of Iskenderun (!!) in Turkey
Kairouan. Indiana Jones and the lost Ark - as Cairo

Aren't the waterfalls in Indiana Jones and the Kingdom of the Crystal Skull the Iguazu waterfalls (with a skull digitally inserted in)?

According to the Internet Movie Database, The Last Crusade was also filmed at Blenheim http://www.imdb.com/title/tt0097576/locations. I can't think of the scene?

Raiders also filmed at Kaua'i, Hawaii and, apparently, Sousee, Tunisia

For completeness, Temple of Doom locations include Yosemite, Macao and Kandy:

Euloroo:
Kaua'i, Hawaii

This isn't part of the WHS, I guess.
I've added the others!

And I couldn't say for sure that Temple of Doom was actualyl filmed within the Kandy WHS boundary
